Immigration Physicals: A Vital Step Towards U.S. Residency
As part of our commitment to serving the community, we offer comprehensive immigration physicals, an essential step for those seeking permanent residency (green card) in the United States. Our office will complete and provide Form I-693, Report of Immigration Medical Examination and Vaccination Record, required by USCIS.

Scheduling Your Immigration Medical Exam
We recommend submitting your medical exam results with your adjustment of status application (Form I-485). USCIS currently requires Form I-693 (or an allowed partial vaccination record) to be submitted with Form I-485. How long a signed Form I-693 remains valid is set by USCIS policy and has changed; as of June 11, 2025, a form signed on or after November 1, 2023, is generally valid only while the application it was filed with is still pending. Confirm current rules on uscis.gov/i-693. Additional vaccines or workups may be necessary, so call to schedule with time to spare.
What to Bring to Your Medical Exam
- Government-Issued Photo ID: A valid passport or other official identification.
- Vaccination Records: As complete as possible. COVID-19 vaccination is not required for adjustment of status as of January 22, 2025 (USCIS). Other age-appropriate vaccines still apply.
- Medication List: If you're on regular medication or have a chronic condition.
- Special Clearance Certificates: If applicable, for conditions like syphilis or a history of harmful behavior.
- Psychiatric Treatment Records: If relevant, include documentation of diagnosis, treatment length, and prognosis.
- Prior chest X-ray, if you have one.

An immigration physical is the medical exam U.S. Citizenship and Immigration Services (USCIS) uses when someone applies to adjust status to a green card. Q2. Who can perform an immigration medical exam?
USCIS accepts Form I-693 only when the exam is performed by a USCIS-designated civil surgeon (or, if you apply from abroad, a Department of State panel physician). Q5. What other vaccines and labs are part of the exam?
USCIS and CDC set age-appropriate vaccine and screening rules, which can include tuberculosis, syphilis, and gonorrhea testing by age. Bring every vaccination record you have. If all required records are provided, DFW Kidney Care Clinic can complete your forms within 24 hours. Completed forms are returned in a sealed envelope, as USCIS requires. USCIS notes that exam fees are set by the office, and many health plans do not cover all portions of this exam. Ask our office when you call 817-912-5900. We do not publish a price here. Your labs will be covered by your insurance.
